For medical records requests that date prior to February 1, 2024, please contact the PAA office.

For medical records acquired after February 1, 2024, please use the Lexington Clinic Medical Records request form.

Immunization records

The secure Kentucky Immunization Registry (KYIR) Public Portal, a free service provided by the Kentucky Department for Public Health, enables people, parents, and legal guardians to access official immunization records from KYIR. Through this portal, you can access your or your child's official immunization record.
