To request a referral:
Complete the Referring Office Appointment Request. This form will route directly to our Referral Specialist who will coordinate the appointment for you and your patient. You can also upload insurance and clinical documents directly into the online form.

- Lexington Clinic is now using Phreesia Connect to make our process easier for referring physicians. If you are a Phreesia Connect client, you can request your referrals directly through Phreesia.
