New Patient and Financial Policies

To improve the quality and efficiency of your visit at Kentucky Ear, Nose and Throat, new patients can download and complete Patient Medical Information forms prior to your appointment. This will help decrease your wait time when you arrive at our office.

If you choose to download and print the forms from the website, it is necessary to download each of the listed forms. Additionally, please arrive to your appointment early to ensure your paperwork has been submitted correctly.

After Hours

If you need to reach a physician after hours, please call +1 (859) 278-1114 or +1 (859) 624-3615. Leave your name, phone number and the nature of your problem with our answering service, and a physician will return your call promptly. If you need a prescription refill, please have a pharmacy number available.
