Medical Records Request:
As of March 1, 2026, please sign up for the Lexington Clinic Patient Portal. All medical records going forward will be available in the Lexington Clinic Patient Portal.
To request a copy of your medical records, please complete a Release of Information Form and return it to us. Links are provided below. If you cannot print this form, please contact us and we’ll forward a copy to you. Once the form is complete, you can mail or fax this to our main office location. The address and fax number are provided on the form below. We will process your request within 30 days of receiving the release form.
