Medical Records Requests

To request a copy of your medical records, please complete a Release of Information Form and return it to us. Links are provided below. If you cannot print this form, please contact us and we’ll forward a copy to you. Once the form is complete, you can mail or fax this to our main office location. The address and fax number are provided below. We will process your request within 30 days of receiving the release form.

School/Work Excuses

If you need a school or work excuse, please request this at checkout the day of your appointment.

Prescription Refill Requests

You may request a prescription refill by contacting our main office number at (859) 263-4444 during business hours. (Prescription refill requests will not be handled after hours.) In order to provide you with timely service, please have the name of your healthcare provider available along with prescription and pharmacy information.

Also, please be advised that our clinical team is assisting with patient care throughout the day; therefore, if you are unable to speak directly with a clinical assistant, you will be routed to the clinical staff voice mail, where you can leave a message with your prescription refill information. A clinical assistant will return your call by the end of the business day or within 24 hours. All refill requests are reviewed and approved by the healthcare provider, and occasionally it may be necessary for you to return to the office for re-evaluation of your condition before a prescription refill request is authorized.

Pathology Results

You may access your pathology results by logging into our Patient Portal website. Results are generally available within two weeks of your visit. If you have trouble logging into the portal, please call the office. Results will only be given to the patient. Therefore, if you would like another individual to receive your results, you must notify the clinical assistant before leaving the office the day of your appointment.
