High Risk Enhanced Care Clinic/ Extensivist Clinic

Lexington Clinic is seeking a full-time BC/BE INTERNAL MEDICINE, FAMILY MEDICINE OR MED/PED PHYSICIAN to join our brand new Enhanced Care Clinic in Lexington, Kentucky. This is an outpatient-only opportunity.

Job Details:
• Care Coordination: Extensivists act as central points of contact, coordinating care between primary care providers, specialists, social workers, and care coordinators.
• Outpatient Management: They provide intensive outpatient treatments, including some hospital-level services like IV medications, in settings outside of the hospital.
• Transition Management: A key focus is managing the patient’s transition from hospital to home, aiming to prevent setbacks and reduce the risk of readmission.
• Comprehensive Assessment: They assess a patient’s complex medical needs, including psychosocial factors and social determinants of health, to develop a unified care plan
• Support System: They work with a patient’s family and their support system to provide resources and ensure continuous care after discharge.
